Crowns, Inlays, Onlays and Labial Veneers are covered only when caused by decay or injury, and only when the tooth cannot be restored with amalgam or composite filing material.

The total number of cleanings and periodontal maintenance procedures are combined in a 12 month period. 

Restrictions may apply to composite fillings.

very misleading product information and little to no member support.

I have had more trouble with this insurance. I have gone to the same place for dental services for four years. Several times during this time when we go, we have been told that we no longer have coverage with our dentist because our service provider has been switched. We have never been advised of this except when we go to an appointment with the same dentist and insurance, and suddenly, we have no coverage again and have to have it switched back. The lack of communication with this company is ridiculous. Also, my son was dropped when he turned 19, and we found out because we went to an appointment and were told he has no coverage as of a month earlier. I asked the person on the phone... Read More

This company has been a nightmare from the beginning. They assigned me to a dentist that had a terrible reputation and who didn't answer their phone. When I tried to switch my provider because of a dental emergency and I couldn't get in touch with the dentist they assigned me, they wanted me to wait a month. Then they had to have the dentist send a letter to them and when I called to confirm that they received it, they said they had no way to know if it was received. Altogether, I spent 6-7 hours on the phone with them over the course of 3 weeks in order to address my EMERGENCY. I had a crown done on a tooth that had heavy trauma damage when I was 8 years old. I then got a letter stating Guardian wouldn't cover because it was not needed. I have been to 6 different dentists in the past 20 years and they all said I would have to replace the tooth as soon as it started discoloring. I noticed some discoloration so I had it replaced and now Guardian does not want to pay the claim. I asked who the dentist was and they told me they use Licensed Independent Consultants and mine was named Dr, Todd Weller. I will obviously be reporting him to the Dental board once I get an email back for which state he is located in.

Sadly Guardian Dental Insurance has provided one of the worst experiences I have ever had with an insurance company. They claim that they are providing a service, but upon trying to use it we are getting blatant NO as a response. Dentist says tooth must come out and I cannot remove it. Just to go to a specialist that says "Yes I need to remove this". Upon submitting to insurance company it never gets approved. On top of all of this after paying them for the insurance coverage they are constantly acting like they even have a say in the matter. DON'T DO IT I was given information over the phone that a waiting period of 30 days would be sufficient for oral surgery. I joined, waited 30 days, made my appointments, had the procedure done, and was denied coverage because "the policy didn't cover major work for 6 months, or 12 months" ( hard to say what she really meant for this procedure). I VERY SPECIFICALLY ASKED AGAIN AND AGAIN IF 30 DAYS WAS SUFFICIENT. Now I am responsible for lots of money. Guardian was my dental insurance when I worked, and when I joined again as an individual they kept kicking back my regular dentist bills as "no longer a member" IF I COULD WRITE THIS REVIEW 100 TIMES IT WOULD NOT BE ENOUGH.

I pay for a premium plan every week through my employer. I've used Guardian once for a cleaning, dealing with the in network provider was great experience and I didn't have to pay anything. Everything was great until one day I called in to see which cosmetic procedures were covered by request of my dentist. The lady spoke with a foreign accent that I could hardly understand and she kept repeating that braces are only covered for children up to 19. I said ma'am I'm asking about cosmetics, after telling me twice that cosmetics are not covered i said so crowns and teeth whitening are not covered? She said again, cosmetics aren't covered. I have Guardian Anytime dental insurance. Went to one of their listed providers today Mid West dental in Mt. Vernon MO. Was quoted $6,000 for a upper denture and lower partial. They want 4 grand from me and a little less than 2 grand from the insurance company. What's the point of having insurance if it covers so little? I have the HIGHEST available plan for my employers group.

This company offers some coverage at a reasonable price... However should you need to contact them... the customer service is god awful and the billing is a nightmare... Getting information on this billing is even worse.... Hope you like talking to a phone system which hangs up on you more than routing you.

Guardian paid only $331.00 and left me paying $728.00 on a porcelain crown, with no discount...... Not a good dental insurance.
