UnitedHealthcare - Overview

UnitedHealthcare is a large multi-line health insurer that provides health, dental, vision, Medicare Advantage and supplemental coverage to over 115,000,000 people. Based in Minnetonka, Minnesota, the company is sixth on the Fortune 500 list of American corporations. UnitedHealthCare offers both standalone and bundled dental plans that includes zero-deductible coverage for preventive care.

QUALIFICATION

The company’s website offers immediate quotes and enrollment without the necessity of medical underwriting. Prospective customers enter their birthdate, gender, and zip code and answer a single question about tobacco use. The company then generates a list of coverage options displayed in comparative format with premium prices and deductibles. The company does not require personal identifying information in order to obtain a quote. Customers can enroll directly from the website.

PLANS

UnitedHealthcare offers a number of dental plans for people of different ages at different premium and deductible levels. While most of the company’s dental plans cover in-network providers only, it also offers some premium plans that cover non-network providers as well. The company’s plans are to some extent tailored by age, with plans for older people that specifically cover dentures and plans for children that specifically cover orthodontia.

UnitedHealthcare Dental Primary Preferred

Quotations for this plan are based on an application of a 35-year-old woman from Fort Lauderdale who uses tobacco. The plan is in the middle of the range of UnitedHealthCare offers.

The description above states an "annual maximum" of $1000. Is that the max deductible or the maximum amount of the benefit?
